HOOP COLLEGE
Building Smarter Players from the Ground Up
TRAINING AGREEMENT, PROGRAM TERMS, RELEASE OF LIABILITY & MEDICAL 
AUTHORIZATION
This agreement outlines how we work together to give your athlete the best possible training experience. Our goal is 
simple: create a structured, positive environment where athletes grow in skill, confidence, and basketball IQ. These 
expectations help us keep sessions consistent, safe, and productive for every player. By enrolling, you agree to the 
terms below, including the assumption of risk and release of liability in Sections 7–8.
Player and Parent Expectations
1. Player Expectations
Athletes commit to showing up with effort, focus, and a growth mindset. Players agree to: be coachable, work hard 
in every session, complete assigned homework and training, communicate progress through the app (daily check
ins), fill out weekly game evaluations and progress reports, use online training resources, and take responsibility for 
their development.
2. Parent Expectations
Parents play a crucial role in supporting athlete development and maintaining program consistency. You can reach 
Coach Mak by phone or text at (754) 900-9822, or through the Hoop College app — whichever is easiest for you. 
Messages are typically answered within 24 hours (Mon–Fri). For longer conversations, schedule a 10-minute call.
Arrive 5 minutes early for warm up. Sessions start/end on time. Late arrivals do not extend session. Missed sessions 
forfeited unless properly rescheduled.
Hoop College runs age-based group sessions where consistency matters. Rescheduling is limited: not guaranteed, 
only allowed if another same-age group has an open spot that week, otherwise session is forfeited. Requests must be 
submitted through the app or by texting (754) 900-9822 before the session. Athletes cannot move into older or 
younger groups for makeup purposes.
Hoop College trains in a non-pressured environment. No coaching, yelling, or sideline instructions during sessions.
If out of town, you remain a member. Please communicate travel dates in advance.
Policies and Terms
3. Weather & Facility Policies
• Hoop College determines all weather-related cancellations
• If the court is safe, the session runs
• If a parent chooses not to attend a playable session, it counts as a missed session
• Weather cancellations will be rescheduled or credited
4. Payments & Billing
• Payment is due before training begins
• Monthly plans auto-bill every 30 days
• If a payment fails, please update your card as soon as possible
• Training will pause until payment is successfully processed
5. Refund Policy
• All sales are final
• No refunds for missed sessions, late arrivals, or unused packages
• In cases of extended injury or illness, notify Coach Mak — billing may be paused and credits may be 
offered at Hoop College's discretion
6. Media & Privacy
• Hoop College may capture photos or videos for training and promotional use
• Families may opt out by emailing support@hoopcollege.com
• Personal information is never shared
7. Assumption of Risk
I understand that basketball training involves physical activity, including but not limited to running, jumping, 
cutting, shooting, contact with other players, and use of basketball courts and equipment, and that these activities 
carry inherent risks of injury, including but not limited to sprains, strains, falls, collisions, and — though rare — 
more serious injury. I understand that Hoop College coaches will use age-appropriate instruction and supervision, 
but that no amount of instruction or supervision eliminates these risks entirely. I voluntarily assume all such risks, 
known and unknown, on behalf of my child, in exchange for my child's participation in Hoop College training.
8. Release of Liability & Indemnification
In consideration of my child being permitted to participate in Hoop College training, I, on behalf of myself, my 
child, and our respective heirs and legal representatives, release, waive, and discharge Hoop School LLC DBA 
Hoop College, its owners, coaches, employees, contractors, and the host facility (Sunrise Athletic Complex, 
including its staff and premises) from any and all claims, demands, or causes of action arising from my child's 
participation, EXCEPT for claims arising from gross negligence, recklessness, or intentional misconduct, which this 
agreement does not release.
I understand that Florida law limits the extent to which a parent may waive a minor's own legal claims, and that 
nothing in this agreement is intended to waive rights that cannot lawfully be waived on a minor's behalf under 
Florida law.
To the extent permitted by law, I agree to indemnify and hold harmless the Released Parties from any loss, liability, 
damage, or costs (including reasonable attorney's fees) they may incur due to my child's participation in Hoop 
College training, except where such loss arises from the gross negligence or intentional misconduct of the Released 
Parties.
This agreement is governed by the laws of the State of Florida. Any dispute arising from this agreement will be 
handled in Broward County, Florida.
Parents will be notified immediately in case of injury.

I authorize Hoop College coaches to seek emergency medical treatment for my child if I cannot be immediately 
reached, and I agree to be financially responsible for any related medical costs not covered by insurance.
Membership Terms and Acknowledgment
10. Membership Terms
Understanding how your membership works.
Monthly Membership
Hoop College memberships are billed monthly and run until canceled — there is no fixed-length contract. If you 
started with the $1 trial, this agreement takes effect once your first monthly membership payment is charged.
Cancellation
• You may cancel anytime by emailing support@hoopcollege.com — cancellations are handled directly by 
Coach Mak, not self-service
• Cancellation takes effect at the end of your current billing cycle — no additional charges after that
11. Acknowledgment
This document represents the complete and current agreement between Hoop College and the undersigned, and 
supersedes any prior Hoop College training agreements, terms, or policies previously provided.
Completing a Hoop College plan purchase enrolls your child in the training program and confirms your agreement 
to the terms outlined in this document, including the Assumption of Risk, Release of Liability, and Medical 
Authorization in Sections 7–9, on behalf of yourself and your minor child. No separate signature is required — 
purchase and participation in training constitute acceptance of these terms.
